a copy of the student's vaccination

a copy of the student's latest school
report (if any) |

4 passport size photographs |

4 self-addressed stamped envelopes |

(non-refundable) |

A reservation fee of
$550 should be paid once the application is successful. This
fee is deductable from the school fee of the first month. (non-refundable) |
